Support Coordinators assist in building capacity for your necessary support to enable you in understanding the NDIS, your NDIS plan and pursue your goals.
The role of an NDIS COS is to connect you with NDIS providers, community, mainstream and government services. Build your confidence and skills to coordinate your supports.

With this further knowledge we are able to successfully assess your current NDIS plan to establish whether it has been effectively negotiated to maximise your funding in order to support your needs.

UNLOCKING POTENTIAL
Support Coordination addresses participants in their complex supporting life. We orchestrate your plan and liaise with your allied health professionals, LAC and family to holistically unlock your NDIS goals.
We focus on monitoring goal progress, reducing complexities, resolving conflicts and addressing change to ongoing supports provided.
We plan for your future and track your allocated funds and predict trends to see your funds implemented successfully. This is to ensure you are not left with any services that have no funding remaining to support you further until your next plan.

FURTHER ASSISTANCE
Englund Care Support Coordination team are trained in therapeutic crisis intervention and accidental counselling to assist you in your time of need. We are not a crisis service, if you need immediate help contact lifeline on 13 11 14 anytime, anywhere.
Free meet and greet with one of our COS professionals, explore your options and if successful sign a service agreement.
MEET AND GREET
Assess and review your plan to see how we can best utilise your funds to grow your capacity and support you best.
REVIEW OF NDIS PLAN
Hold monthly stakeholder meetings, and make introductions with current allied health members. See how your progression is going in the different areas of NDIS and Health. Make and action a plan with clear roles.
CONNECT
Quarterly assessment and review of NDIS goals. Review whether progression has been made or not, write down reasoning. Complete reports to track progression of goals and discuss next steps.
PLAN ASSESSMENT AND REVIEW
Ongoing support throughout your NDIS plan. Assist in linking supports, relocating supports, providing options available, assist in crises where possible. Track progression of Allied Health services with your goals.
SUPPORT AND REVIEW
Assistance and guidance through the whole admin and submission of documents for NDIS plan review process. Then in-person support, via phone call or online video call for the plan review or change of circumstances.
